Job Description

Job Title: Support Worker /Mental Health Care Practitioner

Reports to: Line Manager

Objectives of The Role

• To work as part of a team delivering supported living and floating support services to vulnerable adults
• Provide person centred support to vulnerable adults, which improve the quality of the individuals’ lives, promotes independent living and is relevant to the individuals changing needs.
• Provide a high standard of care and support to individuals.
• Facilitate and enable individuals to be involved in their own care and support, and to participate in the development of services and the wider organisation.
• Perform to the standards laid down by the CQC, Shine's policies & procedures, Skills for Care, SCIE, GSCC and as required and expected by the professionals and service users we deal with.
• To ensure all other legal requirements are met, such as all relevant aspects of Health & Safety.

Key Features of The Role

• To participate in shift patterns, as required by the service, which may include morning/evening/weekend/waking night/sleepover/lone working/on-call duties
• To provide direct support to service users
• To undertake key working responsibilities with, and on behalf of service users
• To take the role of mentor in assisting with induction and development of new staff
• To assist with service improvements
• To assist with the property management tasks appropriate to the service and service users needs
• To develop knowledge base and skills relating to the specific service user group and to the specific needs of the individual service users
• To represent the company in a professional and positive attitude at all times
• To assist services users in identifying and solving problems that represent a barrier to their recovery
• To communicate and listen at the highest levels at all times
• To complete tasks as assigned by the management team to the best of your ability and within agreed timescales
• To understand the wider objectives of the company and how your performance and attitude contribute to this
